Bringing Care and Companionship to Patients’ Homes
The Hospice of the Calumet Area’s Transitions™ program is ideal for patients unsure about leaving their homes and entering a hospice facility. The Transitions support team provides daily living assistance, companionship, and referrals to community resources. Its staff also helps patients understand their care options to better cope with their life-limiting illness.
