The use of thoughts and words can also be sufficient in certain cases. A gesture speaks better than any object. You must remember to be novel each year.
Using the same flowers or a song or some chocolates, year after year will only irritate her and not the same joy that you desire. Try placing yourself in her shoes and think of the things that she would like as a gift.
If you have any photographs of her and you together or any memories you would like to keep alive forever, even a simple photo frame can do the magic. A card to go along with it and she will cherish it forever. Learn to express yourself in different ways. She loves to be with you, if there is any activity you two can do together, and then let it be a surprise to her. Arrange for doing this activity on the special day and she will be overjoyed.
If she likes balloons, then get her balloons. You might look extremely stupid having a bunch of balloons in your hand, but if it makes her happy, then go for it. You could try some novel ideas such as adding a card with I LOVE YOU written in several languages or make a game out of the card or something like that.
She loves chocolates and cookies any time. Whatever be her age, she is going to succumb to a chocolate. Try to bake her something, if you are good at it. Even if you are not, still try so that she is happy that you at least tried to make something for her. Breakfast in bed can also be very romantic.
Whatever be the romantic gift, make sure that you tell her, out loud, how much you love her. It is the caring which matters and not the gift as such. Make sure the gift is to her liking. If she is asking for something special, try your level best to get it for her.
