Adams–Oliver syndrome (AOS) is a rare congenital disorder defined by a distinct combination of features, including scalp and skull defects, which are classified as cutis aplasia congenita, transverse limb defects, and mottled skin. The manifestations of AOS primarily encompass two significant aspects. Firstly, aplasia cutis congenita is characterized by the absence of skin in specific areas of the body at birth, with this condition typically observed at the vertex of the skull. The extent of the affected area can vary considerably, ranging from small, hairless patches to significant exposure of cranial contents. Individuals diagnosed with AOS often show signs of terminal transverse limb defects. These anomalies may affect the upper and/or lower extremities and can present as shortened digits or various other limb irregularities.
Moreover, patients with AOS may demonstrate mild growth deficiency, resulting in their height falling below standard percentiles. Their skin often displays mottling, referred to as cutis marmorata telangiectatica congenita. In certain instances, AOS is associated with additional congenital anomalies, which may include cardiovascular malformations, cleft lip and palate, atypical renal (kidney) development, and neurological complications such as seizure disorders and developmental delays. The syndrome may also present various vascular abnormalities, including hypoplastic aortic arch, middle cerebral artery anomalies, pulmonary artery defects, absence of the portal vein, portal sclerosis, arteriovenous malformations, and abnormalities of umbilical and renal veins.
The management of Adams–Oliver syndrome (AOS) primarily involves addressing the specific congenital anomalies that affect each individual. In cases with significant defects in both the scalp and cranial bones, early surgical intervention with grafting is recommended to optimize outcomes.
