The partnership will bring PocketCM's purpose-built care coordination platform to My Possibilities' team of direct support professionals and care coordinators at their 20-acre Campus for Higher Learning in Plano, Texas.

Founded in 2008 by three mothers seeking better outcomes for their children, My Possibilities has grown into a nationally recognized model for IDD education. The organization provides full-day, year-long continuing education, vocational training, and job placement services — the first program of its kind in North Texas — currently serving over 450 HIPsters (Hugely Important People) weekly.

Under the partnership, PocketCM will deploy its AI-powered case management platform across My Possibilities' care coordination operation, enabling direct support professionals to streamline shift documentation, maintain compliance records, and manage individualized support workflows with significantly less administrative friction.
